The mechanism that kills freedivers and spearfishers: a loss of consciousness that arrives without any warning the diver can detect.

Loss of consciousness from low oxygen during a breath-hold, most commonly during or just after ascent. It gives the diver no warning of any kind, because the urge to breathe is driven by carbon dioxide rather than by oxygen. An unconscious diver underwater with nobody watching does not recover.

What reduces it: Formal training with in-water supervision before diving unsupervised. One up, one down on every dive, with the surface diver watching the whole ascent and staying with the diver for at least thirty seconds afterwards. Never hyperventilate. Surface intervals substantially longer than the dive. Never extend a dive for a fish.

Worse when:

  • After hyperventilation, which removes the warning without adding oxygen
  • During and immediately after ascent, as pressure falls
  • With repeated dives and short surface intervals
  • When there is a fish on the spear

Precautions that always apply here

  • Never spearfish or freedive alone.
  • Dive one up, one down: one diver down, one on the surface actively watching, every single dive.
  • Never hyperventilate before a breath-hold. It delays the urge to breathe without adding oxygen, which is precisely the mechanism behind shallow water blackout.
  • Tell somebody where you are going and when you expect to be back.

This is the single most important thing anybody entering spearfishing needs to understand, and it is counter-intuitive enough that it has to be explained rather than asserted.

The urge to breathe is driven mainly by rising CARBON DIOXIDE, not by falling oxygen. Those two normally track each other, so the urge to breathe arrives while there is still plenty of oxygen — which is why holding your breath at a desk is uncomfortable long before it is dangerous.

Breath-hold diving breaks that coupling in two ways. Hyperventilating before a dive blows off carbon dioxide, which delays the urge to breathe without adding any meaningful oxygen; the diver then has a longer, more comfortable dive with less warning. Diving-medicine guidance describes hyperventilation as working precisely by compromising or obliterating the body’s natural protection, and treats no more than about two full breaths before a dive as the cautious limit. And on ASCENT, the falling water pressure reduces the partial pressure of oxygen in the lungs, which can drop it below the level needed to maintain consciousness — sometimes very suddenly, in the last few metres.

The result is a diver who feels fine and then does not exist. There is no warning, no distress, and no opportunity to self-rescue, because the person is unconscious.

The response is not a technique the diver performs. It is a person watching them: one up, one down, on every single dive, with the surface diver positioned to reach and recover them. That, plus proper training and never hyperventilating, is the whole of what is known to work.

How this drawing was produced

Drawn in Programme 59 as the first non-tackle figure in the catalogue. It states no depth, no time and no recovery interval, because those are training decisions an instructor makes for an individual and this catalogue does not hold them — the drawing carries the RELATIONSHIP between the two divers and the prose carries everything else. RENDERED AND INSPECTED. The watch bracket deliberately extends past the point where the dive profile returns to the surface, and the gap between those two ends is the entire argument of the figure; an earlier layout ended both at the same x and the diagram said nothing prose did not.

Why the buddy is not optional

Almost every safety practice in fishing reduces the chance of something going wrong. The buddy system in breath-hold diving does something different: it is the only thing that changes the outcome once it already has.

A blackout underwater is survivable. An unconscious diver brought to the surface promptly, with their airway kept clear, will very often resume breathing on their own. What kills people is not the blackout; it is the minutes spent underwater afterwards with nobody there.

That is why the protocol is specific rather than general. It is not 'dive with a friend'. It is ONE UP, ONE DOWN: one diver in the water, one on the surface actively watching that diver's entire dive and ascent, on every dive, alternating. Two people diving together at the same time are two people diving alone.

The surface diver watches the ascent particularly, because that is when blackout most often occurs, and stays with the surfacing diver for at least half a minute afterwards, because a blackout can happen in the first seconds at the surface too. What they are watching FOR is worth knowing: a large release of bubbles from the mouth is the recognised sign, and a diver who does that is a diver you go and get.
