Surge, and how it behaves around structure

Swell arriving at the coast becomes back-and-forth water movement that amplifies near the bottom, in gullies and against rock — and it is surge rather than the swell itself that puts divers into the ground.

Surge moves a diver bodily against rock, amplifying in gullies and against faces, and is most dangerous at rock entries and exits where a person is half out of the water and cannot resist it.
What reduces it: Checking the exit before entering and identifying a second one, watching several sets to learn the rhythm, moving in the lull and committing, ending the session early when swell is building, and not entering at all if the entry cannot be made calmly.
A swell in open water is mostly circular motion of water particles with little net movement. As it reaches shallow water and meets the bottom, those circles flatten into back-and-forth movement. That is surge, and a diver in it is moved bodily forwards and backwards several metres with each wave.
WHERE IT AMPLIFIES. Surge is strongest where water is forced into a confined space: gullies, channels, arches, the gaps between boulders, and against vertical rock faces. A modest swell offshore can produce violent surge in exactly the terrain that holds fish.
WHY IT IS DANGEROUS. Not because it is strong, but because it is uncontrollable. A diver in surge cannot hold position, cannot avoid a rock face that is approaching, and cannot make fine movements. Injuries are impacts with the ground: hands, knees, head, and equipment. And a diver working inside a hole or under a ledge in surge can be moved into a position they cannot easily reverse.
WHAT IT DOES TO VISIBILITY. Surge stirs sediment off the bottom continuously, which is why a mark can be clear at depth and unusable in the shallows on the same day, and why visibility falls as swell builds.
AND IT ARRIVES IN SETS. Wave energy comes in groups, and the largest wave of a set can be several times the height of the ones before it. A period of calm is not evidence of calm conditions — it is the gap between sets.
The entry and the exit, which is where surge actually hurts people
Surge underwater is uncomfortable and manageable. Surge at a rock entry or exit is where the serious injuries happen.
The mechanism is simple: a person half in and half out of the water, holding equipment, on a wet uneven surface, being moved by a force they cannot resist. The swell that lifted them onto a ledge withdraws and leaves them, or arrives while they are climbing and pushes them into rock.
THE EXIT IS THE HARDER HALF and it is checked before getting in. Swell builds through a session more often than it drops, and an entry made in a lull may be an exit that has to be made in a set. A diver who has not identified a second exit has planned one option.
And the timing is the technique: watching several sets to learn the rhythm, moving in the lull, committing fully rather than hesitating on the edge. That is a skill better learned from somebody local than from a page, which is one of the practical arguments for finding people to dive with.

When surge means not diving
If the entry cannot be made calmly, the dive does not happen. That is the cleanest threshold in this whole family, and it is available before anybody is committed.
If surge is strong enough that a diver cannot hold position near the ground, the fishing has stopped being possible anyway and the injury risk has risen — so continuing achieves nothing.
If swell is BUILDING, the session ends earlier than planned rather than later. Swell that has built while a diver was in the water is the specific situation in which an exit becomes impossible, and the diver's own assessment is degraded by cold and fatigue by then.
And a general note for anybody diving an unfamiliar coast: a groundswell arriving from a distant storm can produce large waves on an otherwise calm, windless day. The absence of wind is not evidence about the swell, and forecasts that report wind and sea state separately are worth reading in full.
