Cold water, and how quickly it degrades judgement

Cold takes capability away in a specific order — dexterity, then thinking, then shivering — which means a diver loses the ability to make good decisions while still feeling capable of making them.

Hypothermia — Can cause serious injury or death
Cooling removes fine motor control and then cognitive function before it produces shivering or a feeling of being cold, so a diver loses the ability to make good decisions while still believing they are making them — including the decision to stop.
What reduces it: Planning session length in advance and treating it as a limit, ending the session at the first loss of hand function rather than at the first feeling of cold, adequate exposure protection with weighting matched to it, a buddy briefed to call it, and warming properly afterwards.
Cold water shock — Can cause injury needing treatment
Sudden immersion in cold water triggers an involuntary gasp and uncontrolled breathing for the first minutes, which is a drowning risk and is a separate mechanism from progressive cooling.
What reduces it: Entering gradually where possible, allowing breathing to settle at the surface before the first dive, adequate thermal protection, and never taking a first dive immediately on entry.
Cold is the hazard most often described in terms of comfort, and comfort is the least important thing about it.
THE ORDER MATTERS MORE THAN THE TEMPERATURE. As a diver cools, the losses arrive in a sequence that is consistent enough to plan around.
FIRST, PERIPHERAL DEXTERITY. Hands and feet cool fastest because the body reduces blood flow to them to protect the core. Fine motor control goes early — and the things a diver may urgently need to do are all fine motor tasks: operate a quick-release buckle, use a knife, clear a tangle, reload, hold a float line.
SECOND, AND THIS IS THE DANGEROUS ONE, COGNITIVE FUNCTION. Thinking slows, attention narrows, and decision-making degrades. The critical property is that this is not accompanied by a feeling of impairment. A cold diver does not notice being cognitively cold; they notice the water, if that.
THIRD, SHIVERING AND OBVIOUS DISTRESS. By the time these arrive the first two have been happening for a while.
SO THE PRACTICAL RULE INVERTS THE INSTINCT. Waiting until you feel too cold to continue means waiting past the point at which you were still making good decisions about whether to continue. The decision has to be made on a schedule, or on the first sign of lost hand function, rather than on how it feels.
COLD WATER SHOCK IS A DIFFERENT THING and is covered separately. That is the involuntary gasp and hyperventilation on sudden immersion, it happens in the first minutes, and it is a drowning risk rather than a judgement one. Both matter; they are not the same hazard and the responses differ.
AND THE COMPOUNDING FACTORS. Wind on a wet diver at the surface. Repeated dives without warming. Long swims. Inadequate or wetted-out suits. Fatigue. Nothing to eat. Each of these accelerates the same sequence.
The signals worth acting on, and the ones that arrive too late
ACT ON THESE.
LOSS OF HAND FUNCTION. Fumbling a buckle, struggling to hold a float line, not being able to feel the trigger. This is the earliest reliable signal and it is directly a safety one: a diver who cannot operate their weight belt has lost the mitigation that keeps them at the surface.
HAVING TO THINK ABOUT SOMETHING THAT IS NORMALLY AUTOMATIC. Route back, tide state, where the exit is. Slowed processing shows up here before it shows up anywhere the diver can feel it.
A BUDDY SAYING SO. Cognitive impairment is far more visible from outside than from inside, which is one of several reasons the buddy is watching more than the dive.
DO NOT WAIT FOR THESE.
SHIVERING, which is late. Feeling genuinely cold rather than uncomfortable, which is later. Any confusion or unusual behaviour, which is later still and is an emergency rather than a signal.
AND THE RESPONSE IS ALWAYS THE SAME. Get out. Not one more dive, not a quick look at the far side of the reef. This is the single clearest case in the discipline where stopping is the whole of the correct answer, and where a diver who is already impaired is the worst possible judge of whether it is necessary.
WARMING AFTERWARDS is unglamorous and matters: out of the wind, out of the wet suit, dry layers, warm drink, something to eat. A diver who gets straight into a car and drives while still cooling is continuing the exposure.

Why cold makes every other hazard worse
Cold rarely appears alone in an incident account. It appears as the thing that made something else unmanageable.
WEIGHTING. Thick suits are buoyant, so cold-water diving requires more lead, so a weighting error costs more — and a cold diver is the one least able to operate the release.
ENTANGLEMENT. Clearing a tangle is a fine motor task performed on a finite breath, and it is the first capability cold takes.
BREATH-HOLD. Cold shortens usable breath-hold and increases the work of swimming, so the same dive costs more oxygen than it did an hour earlier. A diver working to their usual times as they cool is working closer to the edge without knowing it.
EXIT. Getting out over rock or up a ladder is a strength and coordination task at the end of the session, which is when a diver is coldest and most tired.
RESCUE. A cold buddy is a worse rescuer, and two cold divers is a much worse situation than one.
The implication is that cold should be treated as a session-level constraint rather than as a per-dive one: it decides how long the session is, not how any individual dive goes. Planning the session length before getting in, and treating that plan as a limit rather than a target, is the whole of the mitigation.
